Driver Abstract

A driver's abstract must be requested in person with valid ID and can be printed as requested for 3, 5 or 10 years.

Third Party Abstract Request

An individual who is personally known to the driver who has written consent may apply in person for an abstract on the driver’s behalf. The person who is requesting the abstract of their driving record must fill out a Driver Abstract Consent form and sign it. The individual coming in to pick up the abstract on their behalf must fill out the Driver Abstract Statement of Intent and present acceptable ID.
